At Carefirst Rehab Home Health Services, our licensed and experienced therapists, nurses and home health aides will work with you, your physicians and your family to develop an individualized plan of treatment specific to your needs and meet your personal goals and achieve your highest level of potential.
Home Therapy and Rehabilitation
Our Physical, Occupational and Speech Therapists provide in-home therapy services to expedite the recovery process from illness or injury. Our highly-skilled Physical and Occupational Therapists will come to your home to provide therapeutic exercises, balance improvement, gait training, and instruction on the use of assistive devices.
Improving Speech and Swallowing
For some patients, speaking and swallowing problems can be vital to their overall health and communication ability. Through performing swallowing evaluations and recommendations to modify diet and food intake, our speech therapist is able to promote better quality of living for our patients in the home.
Recent Stroke Recovery
After surviving a stroke episode, most may experience decreased mobility on the affected side of their body. Our clinicians will work with you or your loved one to help you progress towards your goal of regaining your previous functional level and/or achieve your maximum rehabilitation potential, through performing and recommending various therapeutic exercises that you can do in the comfort of your own home.
Cardiac Rehab
In-home cardiovascular rehabilitation aims to help individuals who are suffering, or those that are at risk, from heart disease through performing therapy exercises focusing on reducing the risk of future cardiovascular episodes and ultimately improve the patients’ overall health. We will collaborate with your family members, caregivers, doctors and other healthcare providers involved in your care to generate a plan of treatment specific towards your condition.
Pulmonary Rehab
For patients with Chronic Obstructive Pulmonary Disease (COPD) and other lung-related diseases, pulmonary rehab can be beneficial for patients as it aims to help improve overall pulmonary function. After discharging from an inpatient setting, we can continue pulmonary rehab at home through continuously educating the patient about the disease, applying breathing techniques, teaching energy conservation techniques and performing exercise training.
Nurse On Call
Our primary purpose is to give you the quality skilled services you need. We offer a 24-hour phone line so you can contact us for any medical need. You may contact us any time should you have a clinical question or have an emergency. Our Registered Nurse will be there to take your call.

Maintenance
Maintenance therapy is a form of intervention that has a focus towards maintaining, preventing or slowing down unnecessary decline in a patient’s health condition and functional level. The goal of a maintenance program is to minimize the impact of a medical condition. Under this therapy program, our qualified therapists will assess the patient in the home and establish a maintenance program fit for the patient’s needs.
Wound Care
Our clinicians are wound care trained. We want to ensure that you receive the special care and treatment your doctor recommends. Our clinicians will come to your home to provide the necessary wound care that you need, as ordered by your doctor. We will train and educate your family members and/or caregiver/s and provide all information required to help heal your wound/s and to prevent any new wound formation and potentially decrease hospital stays.
Pain Management
Our therapists and nurses work hand-in-hand to provide patient education, instructing the patient on proper ways to use medications, including pain medications, at home. Our clinicians are also trained to assess the patient’s level of pain by asking a series of standard questions to accurately measure it. Vital signs will also be monitored regularly.
Medication
We understand the significance of being compliant with taking medications to our patients’ health. With Carefirst Home Health, our clinicians are trained to perform ongoing medication monitoring to determine their actions, indications, special precautions, patient’s response to therapy, side effects, allergies and contraindications, and to ensure appropriateness and continuity of care and to identify the potential complications from the therapy being provided.
